2024-2025 Season Registration Form
Registration Fee: $50 per player
(Please bring cash or a check made payable to "Lakeview Basketball Association" to the registration meeting October 13th or VENMO @lakeviewbasketballassociation)
Volunteer Deposit: $100 per player

* Scholarships are available for candidates that meet financial guidelines. Forms are available upon request.
Scholarship grantees will be based on the decision of the LBA board.

The undersigned, as a parent/guardian of the minor child, give my permission for the player named above to play on the Lakeview Basketball Association (LBA) traveling team. I acknowledge that playing on the traveling team could lead to injuries because of the factors inherent in this type of activity. I accept the responsibility for any medical treatment and its expenses. I understand that the people associated with the LBA board, coaches, parents, and financial supporters are exempt from any liability or blame for any and all injuries, accidents and/or damages which might occur during ordinary course of the activity.
I authorize and consent to any x-ray, examination, anesthetic, medical or surgical diagnosis or treatment to be rendered to my minor child under the general or special supervision physician or surgeon licensed to practice in the healing arts, when the need for such treatment is immediate and when efforts to contact us are unsuccessful.

: I understand that I am responsible for all issued equipment, the up keep while in my child's possession, and will return of such equipment at the end of the season. I understand that I will be charged an additional fee if issued equipment is not returned in satisfactory condition.

PARENT & SPECTATOR CODE OF CONDUCT
:
● Parents should conduct themselves responsibly and show good sportsmanship. ● Accept willingly and graciously the decisions of the officials. ● Remember that children participate in athletics to have fun. ● Remember that the game is for the children, not for the adults. ● Remember that they are not the coach and will refrain from yelling instructions out onto the court. ● Speak to the coach in private with any concerns, never in front of the children or on the court. ● Be a positive role model by respecting all players, coaches, referees, parents and spectators at all times. ● Value skills development, fair competition and good effort. Recognize that the efforts of all children, your own team and opponents, are more important than the outcome of the game. ● Cheer for all children on the team in a positive fashion, not just your own child. ● Refrain from rude or obnoxious behavior or comments toward the opposing team and officials, and never confront referees or other game personnel. ● Not encourage any behaviors or tactics that would endanger the health and wellbeing of anyone. ● Be informed about the rules and regulations of basketball. ● In the event of questions or concerns, please contact a LBA Board Member.
